Talk to your current physician or find one near you and have them fill out and mail into the Department of Health your Illinois physician written certification form confirming that you currently have one of the qualifying conditions.
Remember: Your appointment must be within 90 days from the day you submitted your application.
You must have the following documents in order four you medical marijuana application to be approved. A recent headshot, two proofs of Residency, proof of age and identity (valid gov ID), and your fingerprint consent form and receipt.

As of August 28th, 2018 Fingerprints are no longer required.
Fill out the Illinois Medical Cannabis Pilot Program application and choose the medical marijuana dispensary you wish to purchase cannabis products from. You can also select a caregiver to help deliver your medicine to you if you do not need a caregiver simply skip that section.
Future patients looking to purchase a three-year cannabis ID card can apply online. Two and one-year registry card applications must be mailed in.
The application fee differs depending on the number of years your card is valid. A one-year registry card cost $100, a two-year registry card costs $200 and three-year registry cards cost $250.
Veterans and patients enrolled in either the Federal Social Security Disability Income (SSDI) or the Supplemental Security Income (SSI) programs qualify for a reduced application fee consisting of: $50 for a one-year registry card, $100 for a two-year registry card, and $125 for a three-year registry card.
Now that you have gathered all your required documents and completed the Illinois medical marijuana application you are ready to mail in your application to the Illinois Department of Public Health. And that’s it you have now successfully submitted your medical marijuana application.
Once you have applied and been approved for Illinois medical marijuana you will receive an Illinois medical cannabis patient identification card. After receiving the card you will have the opportunity to choose a dedicated Illinois medical marijuana dispensary (form located here) which you will purchase Illinois medical marijuana at on a regular basis.
