Illinois Medical Cannabis Pilot Program
The Illinois Medical Cannabis Pilot Program began in January 2014 and is scheduled to sunset at the end of 2017. Illinois medical marijuana patients hope that this program will continue past 2017 but it is ultimately up to state government to extend the Illinois Medical Cannabis Pilot Program. Under the current law, Illinois medical marijuana patients and caregivers can legally purchase and possess medical marijuana for the use of treating one of the Illinois medical marijuana qualifying conditions. Patients who suffer from additional conditions that are not currently on the Illinois medical marijuana qualifying conditions list have the opportunity to petition in order to have their condition added to the list.
Illinois Department of Public Health
The Illinois Department of Health regulates the patient and caregiver applications as well as the certifying physician approval process. Illinois medical marijuana can only be legally obtained by certified Illinois medical marijuana patients who have been approved by the Illinois Department of Health. In order for a patient to qualify for medical marijuana in Illinois they must suffer from one of the qualifying conditions (click here for list) and a certifying physician, in which they have a bona-fide relationship, must write a recommendation via Certifying Physician Form. The Illinois Department of Health also manages the Illinois medical marijuana patient and caregiver ID cards.
Qualifying for a medical marijuana card in Illinois
In order to qualify for medical marijuana in Illinois a patient must be a resident of Illinois and over the age of 18. Illinois medical marijuana patients must suffer from at least one approved medical condition. If you suffer from one of the Illinois Medical Marijuana qualifying conditions you must visit a physician to seek a written certification for Illinois medical marijuana. In order for a physician to be eligible to recommend medical marijuana in Illinois they must be a doctor of medicine or osteopathy. Illinois physicians who recommend medical marijuana must also be in good standing to practice medicine in Illinois and have a bona-fide relationship with the patient they are certifying.
Illinois Medical Marijuana for Minors
Minors are eligible to become certified for medical marijuana in Illinois. In order to use medical marijuana in Illinois, minors must obtain written certification from two Illinois licensed physicians. These physicians must use the Physician Certification Form as well as the Reviewing Physician Certification Form. In addition, Illinois medical marijuana patients under the age of 18 must be attached to a caregiver. A patient’s parents can also serve as their caregiver.
Certifying Physicians for Illinois Medical Marijuana
In order for Illinois physicians to recommend medical marijuana in Illinois they must be either a medical doctor (M.D.) or a doctor of osteopathy (D.O.). Illinois medical marijuana physicians must also be in good standing with the State of Illinois and hold a DEA license. Physicians in Illinois who recommend medical marijuana are not required to undergo any additional training in order to write a recommendation for medical marijuana. Although there is no special training needed, physicians are required to have a bona fide relationship with the patient that they are certifying. Because medical marijuana is still federally illegal and considered a Schedule 1 controlled substance, physicians are not prescribing medical marijuana; instead they are recommending it for their patient. When a physician determines that medical marijuana can help a patient treat their qualifying condition they must fill out a Physician Certification Form.
Illinois Medical Marijuana Qualifying Conditions
There are currently 37 conditions which qualify patients for Illinois medical marijuana. If you suffer from one of these conditions, you have the opportunity to use medical marijuana in Illinois to treat your debilitating condition. If your current treating physician is open to recommending medical marijuana for your qualifying condition you can fill out either the Online Patient Application or print out and mail the Hard Copy Patient Application. In addition to the Illinois medical marijuana patient application, your treating physician must fill out a Physician Certification Form. Once your application is completed and the Illinois Department of Public Health has approved your Illinois medical marijuana application, you will receive an Illinois medical marijuana card.

Illinois Medical Marijuana Physician Certification
In addition to submitting the Illinois medical marijuana patient application, the certifying physician must also fill out and send in the Illinois medical marijuana certifying physician certification form. This form can be found for download here. You can direct your referring physician to this page to download the form or you can print it out and bring it to your physician to fill out and send in.
Choosing a Medical Marijuana Dispensary
Once approved, Illinois medical marijuana patients must choose the dedicated Illinois medical marijuana dispensary that they will purchase medical marijuana from.
